We have established what we feel to be reasonable and competitive fees for our services.
New Patient Examination and history ranges from 43.00 to 95.00 depending on the severity of the injury or complexity of the problem. The testing is required in determining the correct diagnosis and devising your treatment plan.
Chiropractic Adjustment is 40.00
Any other charges will be discussed with you before the service is rendered.
Insurance Billing
We participate with many health care plans, which aid in the payment of your health care costs. Current insurance cards should be presented to Susan at the time of service. We will call your carrier to determine your chiropractic benefits.
We would like to emphasize that as health care providers, our relationship is with you, not your insurance carrier. Insurance is a contract between you and your insurance company. We will be happy to bill your insurance and follow up on your claims, but you are ultimately responsible for your account at our office.
Co-payments and deductibles are due at the time of service.
We are Blue Cross and Lifewise Preferred Providers.
Medicare
We are participating providers for Medicare. You must have part B to be eligible for chiropractic coverage. Medicare has a limited benefit of what it will cover for chiropractic services. The only allowable treatment charge for chiropractic is chiropractic manipulative treatment(CMT). Medicare requires an examination but will not pay for it.
Personal Injury
If you are the injured party and own the automobile, we will bill the medical insurance portion of your own automobile insurance policy (PIP).
If you were a passenger in someone else’s car, we will bill the driver’s auto insurance policy.
If you were a passenger in a vehicle which was not insured, but you own a car which has medical coverage, the insurance company which carries your policy will be responsible to pay your medical bills.
If you were driving on an errand for your employer, or driving on company time, this is a worker’s compensation case NOT a personal injury case. We will bill your worker’s compensation carrier. Third party settlement (disability, lost time, car repair) may happen after treatment is finished.
If you are a medicare patient and there is other insurance available, you may not bill medicare for your personal injury claim.
• We accept cash, checks, Visa and Master Card. There will be a $30.00 charge on all returned checks.
• Patient statements are billed monthly and are due upon receipt. We understand that problems arise that may require some assistance. Contact us and ask for Susan.
• Any account at this office over 60 days will be subject to finance charges.
